2025-2026 Student Athlete Required Documentation

Pre-Participation Physical
Physical paperwork Will be sent out through Teamworks, including Pre-Participation Physical Evaluation, Medical Consent, Concussion Responsibility Statement and Sickle Cell Testing. All required documentation must be submitted in order to sign up for a physical slot.

Physicals must be completed by the JCU team physician. On campus Physicals:

• Saturday, August 2, 10-2 *Appointment only
• Tuesday, August 12,  9-5 *Appointment only
  • If you’ve had any previous surgeries, cardiology testing, neurological disorder, please be sure to provide all testing and documentation with you to your physical to not delay your clearance of participation in your sport.
  • On the day of physicals:
    • You will have your heart rate, blood pressure, height and weight checked.
    • You will have an Electrocardiogram or EKG performed by medical staff at no charge (required for all incoming JCU athletes.)
      *if further testing or evaluation is required any fees or costs associated will be the responsibility of the student-athlete
    • You will complete an additional concussion baseline using an iPad.
    • You will meet with a Cleveland Clinic Primary Care physician, for the medical portion of your exam
    • You will meet with a Cleveland Clinic Orthopedic Surgeon for the musculoskeletal evaluation.  
      • Please DO NOT workout 1-2 hours before your scheduled time
      • Please do not consume energy drinks or excessive amounts of caffeine/ pre-workout supplements, etc.
      • Avoid wearing lotions
      • Wear comfortable clothing, workout clothing, sports bras, shorts, t-shirts

Insurance
Please provide a copy of your primary health insurance card. (Front and Back)

John Carroll University provides a secondary insurance policy for incidents that may occur during varsity sports practices or competitions.  Your family/personal insurance policy continues as the primary coverage.  If you have any questions concerning the insurance policy, please feel free to contact Garry Homany at (216) 397-1982 or Don McPhillips at (216) 397-4503.

Concussion Baseline Testing

1) IMPACT TESTING, Pre-Concussion testing. Log into to following website:

NOTE:  The link above for ImPACT can ONLY be accessed through Internet Explorer (i.e. NOT CHROME). 

DO NOT SKIP DEMOGRAPHIC PORTION.

Click on Baseline Testing and follow the prompts.

Under the Supervision question, select UNSUPERVISED and follow the prompts. While taking the test minimize ALL distractions (No phone, no text, no music, etc...) Additionally, please make sure all "pop up" blockers are "off".  Please read ALL directions carefully and understand them before beginning the test. Use a mouse. This test may take up to 30 minutes to complete. Testing must be complete prior to completing your physical at John Carroll.
